Hi. I've recently started using jotforms and finding them a great resource.
My issue is - I used a template and edited some of the questions to create a reference request form. When I send the link the form has my questions on but when I receive it back it has these questions on it:
Do you consider the candidate to need Clinical or Behavioural Development?
Are you aware of any suspicious proceedings, Policy investigations or disciplinary action?
These questions are totally inappropriate for my use, how do I get rid of them when I can't see them on my form?
I've sent link to the form where you don't see the two questions and a small section of the form returned to me with the two questions. I couldn't send more information as it's confidential. How is this happening?

What I understand, you just need to edit email template with appropriate questions, that's it. Please review the following screenshot on it;
