I checked your form and it does indeed appear to be blank. I checked your forms revision history and there were no changes made recently that would have cause this. We were having some issues reported by other users today regarding blank forms but most of the were able to be fixed by using the revision history:
However, I could not find a previous point to restore your form to.
I have forwarded this to our developers to see if they can restore your form. My apologies, we hope to have this resolved as soon as we can.
