The only issue I'm having right now is with a single user - a new customer. They are on IE8 and reporting that not all of the attachments they are uploading are going through to us on our end.
At this customer's request I even added the additional 4 upload fields (total of 8). She appears fairly knowledgeable and understands it might be a IE8 compatibility issue. I told he I'd send it through to IT (you guys) and get your thoughts.
In meanwhile she'll try Google Chrome and/or Firefox to see if that resolves issue. I looked through your knowledge base and didn't see anything current (2012/2013) on IE8 compatibility so I reach out to your for assist please.

Are they getting any warnings or errors in IE8?
Your form looks good, and as far as I know, we do not have any major issues with the form upload fields.
I have also tried to reproduce the issue using IE8 via https://www.browserstack.com, but all the files were uploaded fine.
