Why am I getting two different form impressions?

    The form  I created "ECA Life Application" is not revealed in preview mode or in my website.  i copied the embed code into my site and get something totally different...the preview mode form.  Why am I getting two different forms?  How do I correct it?

    I maybe checking the wrong problem, so please correct me if I did.

    When I checked the form in your website, the form I see on the webpage was exactly the same to the actual form http://www.jotform.us/form/50508914228152 .

    Can you please described more how we can check on the "difference" you mentioned.
