    All of my forms have been disabled from my website: www.healthandsafetyweek.com

    Hi, thanks for replying. i have upgraded the membership but the form is still disabled. please can you help?

    That form, http://www.jotformeu.com/form/51595021568357, seems to have been marked as deleted - perhaps accidentally. I have logged into your account, and restored it from your:

    My Forms page > Trash folder > clicking on the form > Restore
