    My form is gone

    I have 29 submissions on my form and when I click to see the submissions it says that there aren't any... I also have this form embedded into my webpage and it's not showing up, I cannot view or edit the form in any way. 

    The link to this form had been mass emailed and is now dysfunctional. I am losing money, and appearing inadequate... please help

    Sorry for inconvenience you have.

    I have forwarded this issue to our developers, so that they can take a look into why your form is appearing blank and why your submissions are not being displayed. 

    In the meantime you can try following this guide on how to revert back to an earlier version of your form where all the edits are there: http://www.jotform.com/help/294-How-to-View-Form-Revision-History

    We will let you know through this thread as soon as we have any more information from our developers.

    I'm sorry for the trouble caused to you.

    This issue is already resolved. I did check your form and it seems to display all form fields. Your form is now working fine. Please check the screenshot below:

     

    I would suggest you to test your form again and get back to us if the issue persists.
