All of the fields in my form disappeared?

Hi,

I was editing my form (http://www.jotform.us/form/51577861418161), saved it with all the fields (probably ~20 or so), and when I revisited it, it was suddenly blank. I definitely did not delete anything. 

Can you tell why or what happened? I really hope this can be recovered.

Your form has been restored to the previous version using form Revision History tool.
