Why did my form disappear?

I created a form titled "2015 June Task Force" and after I made an update to the confirmation email, the form disappeared. What I mean is that I can see the title, but when I try to edit the form, it's blank, but the confirmation email is still there. Also, I did a test submission, and on the my forms page it shows on submission, but when I go to the submission page I can't see anything.

I have used the Revision History to revert the form to a latest working version. Please try to edit it now. 

There are some problems on the form builder. We are looking into it.
