About the preview of my form

Goodafternoon,

i have make some forms but every time i press the preview a message appears that it says: 

"form not found, this form is disabled" but i have check that they are enable and i dont know what's going on here..

I would appreciated if you could help me with that.

I checked your account and it seems that your account has been automatically suspended by our system because your forms are suspected to be used for phishing since it contains password fields. Please take note that JotForm does not support membership features such as login forms with passwords. Such forms will be detected phishing and be suspended.

I have activated your account but I suggest you to remove the password fields from your forms to avoid your account being suspended again.
