I went ahead and using the form link you provided above http://form.jotform.us/form/50644011432138 , created a fillable pdf and had some of the issues you explained, pages not breaking where they are supposed to, long blank spaces, no margin.
It seems the issue was due to the size as it is a multi pages form and the page breaks as well.
Can you please try to remove the page breaks from the original form and create again a fillable PDF form?.
I am submitting a request to our next level team, in order for them to look deeper into this issue and find a solution.
We will contact you through this same thread as soon as a this is solved.

I also made a test using your jotform http://form.jotformpro.com/form/51606273666964 and converted it to fillable PDF Form.
This is the result PDF form
as you can see, it is not as distorted as described it must be.
It will also help our developer assigned to fix the issue if we can provide them more information.
Will you be able to share to us the PDF created on your end? If you can show us also a screenshot of the result, it will greatly help.
At this time the ticket was already on the hand of our dev. They will notify you here for any updates/news on the status.

Sorry for the late response. We have made several improvements during this time on PDF forms. You should be able to create a better one now. Thanks for your patience.
