Changes to Form from Template Disappeared!!

Please help, I've been working on a form for several hours, which I was adapting from the "employee information" template.

When I just went in to edit it, it has reverted back to the original template, with none of the changes I made, even though I saved many times! Where did my form go? This was almost three hours worth of work, just gone!

I was just playing around with creating a Zapier "zap" to integrate the form with Trello, could this have been the cause?

I have found the third "employee information" form under a guest account (cookies based account) associated with your IP address. The issue should be fixed now, the form has been moved to your registered cayleigh account.
