I set up notification to go to email@example.com, but I am not receiving them from the form submitted to http://form.jotformpro.com/form/51586007702957. Also, please confirm that the person that completes the form is receiving a confirmation email (not just the thank you) with their data, after submission.

I checked our logs and since you have changed from using "firstname.lastname@example.org" to "email@example.com" the notifications are now going through. Prior to that change, they were failing to be delivered from "firstname.lastname@example.org". Several of our account notifications also seem to have failed. AOL addresses are notoriously bad with blocking notifications. Though your appear to be working now, I would recommend if possible changing to a different address for your notifications.
Your autoresponder is setup properly and should indeed have been sent to the address entered in the "Parent Email" field in your form with each submission. There should not have been any issues with the delivery of those.
If you notifications stop coming through once again, let us know and we will see what other options we can provide.
