Completed pdf looks nothing like the original form.

I notice that the pdf form looks nothing like the form I created.  Is there a way for the completed pdf to look like the original form?

Are you referring to the PDF copy of the submission?

You can try to make it more like your form by: 

Go to the Submission's page.
