I can't select anything when using my iPhone or any mobile device. I t works fine on the computer. I'm using it as a link and it's being redirected to a webpage. Please advise.

I have cloned your form and found out that it is set to be Mobile Responsive within the Designer. But, could you also set it to Mobile Responsive via Preferences.

It's working fine here using my android device, Google Chrome and default browser. Here's a screenshot:
And still working using iPhone IOS 8, Safari browser. Here's a screenshot:
Please try doing the following:
a. Go to the form builder and re-save the form.
b. Clear your form's cache: https://www.jotform.com/help/229-How-to-Clear-your-Form-Cache.
c. Clear your mobile browser's cache.
Let us know here if the issue persists.

Thanks. I figured it out. I'm on iOS 9... I tried it on another phone and it worked perfectly!
