Page Break not working: Clicking next does not open the new page

    The form is online, all fields are showing. However visitors are unable to go to the next page after the page break even after completing all fields. Also the form doesen't throw up any errors. Have tried both the script embed and the source. Any ideas?

    Update:

    Checked the console and it says that prototype.js is undefined (line3). Included it in and it seems to be working now, however form validation is still broken.
