Contact form stopped working

    I have been using your contact form and for some reason it keeps saying that it is disabled.  I was wondering if there is a reason why or if I can get this resolved somehow?

    Hello Lileth,

    I have checked and tested the form on the above inidcated link, and yes, when I hit the Submit button the next screen is Form Disabled.

    It seems the form was removed or disabled from the source.

    I would greatly appreciate it if you could provide us with the form link or original form ID#, so then we will be able to find out the reason why it pops up this message and give you the solution.

    I tried checking the form on your website but the website is not loading

    Form is disabled because your account is suspended. Your account and form was auto suspended by the phishing filter because of the suspected phishing forms you have: 

    Inside of your form you have fields that collect Credit Card information and that is against JotForm Terms of use.

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    You must remove those fields and only then can the account be enabled once again. 

    If you want to have payment fields inside of your form you have to use one of JotForm payment tools.

    I checked the page you provided - - and the form there is working now. I also did a test and successfully submitted.
