Some of the people taking our survey would prefer to get a paper copy of it, but when I click on "Create PDF", the page breaks are in the wrong locations, so the printed version doesn't look like the digital version. Can you help me with this? Our survey is active right now, so I cloned it and below is the URL to the cloned version. Thank you!

It seems that 'Include page breaks' option for PDF forms does not honor form Page Break elements. Mentioned option is simply adding the A4 page breaks automatically (by ignoring the form page breaks). I have forwarded a ticket about this issue to our Development Team.
In the meantime, you might consider using the Print Form button on a blank form.
For example, you can print it into a PDF file with a Google Chrome browser.
We will let you know if we have any updates.

That is because page height of PDF forms are not equal to height of web forms, calculations differ too much indeed while putting fields.
