How to display only desired country in the address field?

Is there a way to NOT include all countries except USA and Canada in the shipping address?

I am not sure if I have understood your question correctly. I did check your form and it seems that you have added address field for shipping address.

Do you mean to say that you want to only display Canada & USA in the list of countries?

Unfortunately it is not possible to remove few country form the dropdown list of address field. One possible solution is to hide the complete "country" dropdown from the address field and manually add a new dropdown question to display only desired country. Please check the screenshot below on how to remove the country dropdown from the address field:
