    Hi,

Your screenshots look like they did not come through completely.  Since I cannot see the differences in your form, I am not quite sure what changes were made.  This is what the form looks like on my end:

Nothing appears to be out of sorts.  The only hidden field in your form is indeed hidden as well.

You can always revert your form back to a previously saved state using your revision history:

If the version you are looking for is not in your history, let us know a bit more detail about what is wrong with your form and we will be happy to help you adjust it.

it's seems that if the html is checked not on-line the form is not deployed correctly.

I check the html reading it from my disk to see if all is correct: doing this the form is not corrected displayed. If I load the same html page on the web server all look ok ...

The strange thing is that this is "new" from 7 days (more or less) before all was ok on and off line ...

The form should not work offline.  Our forms are setup to function only with a connection.  If you are trying to load your form offline, it will only load if there is a cached version of the form available to load.  None of the submission functionality will work offline either.
