This is related to the conversation over twitter ( https://twitter.com/legopuffle/status/610098645342392320 ) in regards to Form Designer first not allowing the entry to any of the forms - which seems to be working at this time, but one form in particular is still not allowing the changes made to it over the Form Designer to be saved, while also not showing all of the fields of the form.

This issue is happening in the latest version of Chrome browser, while the FireFox seems to be displaying it correctly (when checking it out by myself):
