Wondering why the attached screen shot pops up instead of the Thank You screen and the submission doesn not come through.Page URL:

To remove that error, please follow the guide below:
1. Open that form (Edit Mode)
2. Go to Setup & Embed
3. Click Preferences
4. Click Advance Settings Tab
5. Below Advance Settings, Select "No Check" on Unique Submissions.
