I uploaded my form over two weeks ago and cannot believe it is not ready and available yet. Should I look at another option since your group cannot get my form setup and available? Please let me know what I need to do to get this resolved.Also I cannot log into my account is there a password that I should use?

To confirm, you have uploaded a PDF form to your JotForm account, is that correct? I've checking your account and I see that you have a PDF form titled "Old IT Request Form". You also have a similar form with that title but is a web/online form (not PDF).
May I know if you are still missing some forms?
