Format of form from my design is different?

    I have used the direct link in Wordpress to put my form on our website  on 2 pages.  On one page, it shows up but the format is different from my design - rows are messed up.  

    On the other page I get this when I click on the link: Not Acceptable!

    An appropriate representation of the requested resource could not be found on this server. This error was generated by Mod_Security.

    Any idea what I'm  doing wrong?  Thanks.

    Hi,

    Upon checking your link, this is the JotForm URL that I found: http://form.jotform.co/form/51436644412855. May I know how you would like it to be formatted?

    If there are some designs that were quite off, you can simply edit it using our form builder and the Form Designer Tool. If you are familiar with custom CSS codes, you can use it to further make advance styling and formatting of your form.
