Today a customer used the link to my form, filled it out and pushed submit. She said it just brought up a blank page once she did that and I thought I was supposed to be emailed, and I wasn't. Did I do something wrong during the set up process?

Upon checking your form, the first thing I noticed that it has no form title/name, please add one.
After that, I've checked your notification and it is missing the "Sender Name":

Try testing the form by opening the form URL, filling out the form and clicking the submit button. See if you were still unable to receive any notifications after applying all the suggested solutions above.
