Date field is filled in PDF but it is not displayed in the submissions screen

I assume that you're referring to this form: https://www.jotform.com/form/51675968332971. Is that correct?

I made a few tests using a cloned version of that form and what I suggest is to disable the lite mode on the date/time field. 

So, the field would look like this on the PDF file

And the value should be captured correctly on the submissions screen.
