My form - Window Genie Corp Order Form wont load -
Can you help?

Are you still having this issue up to this time? I just checked and test your jotform http://www.jotformpro.com/form/51675960330961 on my browser and it loads just fine.
I did noticed that you seem to have enabled Autofill feature in your form. It may not be necessary to turn this feature on since the form is not that big of a form.
Try also disabling the feature and see if it makes any difference.
Clearing your browser's cache is also an option to try.

Yes - I was getting concerned that I would not get an answer quickly so I re imported the form from Adobe again - that is the form that you looked at - and yes, that one does work, but the alignment is all messed up (this was the same problem with the first time - and you guys fixed it)

I do not care which one we use as long as it looks correct - A bigger question is what happened to the original that it wont load now.

I was able to see the blank/empty form http://form.jotformpro.com/form/50713553994967 (which look like it wasn't loading)
I have run a fixed for it and I can see it now loads properly this time.
Please check again. Let us know if issue still remains.

OK - Got it - However the alignment is still messed up - it does not look like the what you show above.

I made a few changes to the alignment. The label for the field that was a bit off needed to be set to the left side rather than the right:
And merge one field with the line above. Everything appears to be lined up properly though:
