Form was blocked due to Phishing

Hi

I have been using your form for a number of months to collect data and it has suddenly become blocked by our server reference below.

Can you please advise as we need to use form.

I’ve just checked at the following blacklist checking site - http://www.urlvoid.com/scan/form.jotformeu.com/ which shows that the site has been involved in Phishing and as such won’t be unblocked until it no longer appears on those lists.
