I have an intake form I made through JotForm and my patients are complaining that the Submit button is there at the beginning, but disappears once they are done filling the form and actually ready to submit. Any idea what is going on here? Can you help?

Unfortunately, I'm unable to replicate the problem.
I've tested it on desktop browsers Chrome v43, Mozilla Firefox v38 and IE 11. But the submit button still shows properly.
I've also tested it on Android Device.
I've tested it on iOS 8.3
I also can't see any conditional logic that should hide or show the submit button. Is it possible that the button is just small to see? Or the users are viewing it on a mobile device and the submit button is misplaced? Have you tried making the form mobile responsive? You can check this guide on how to do that: http://www.jotform.com/help/311-How-to-make-Forms-Mobile-Responsive.
Please try that and see if it gives you a better result. If you are still getting reports regarding that problem, would it be possible to ask the users what device, OS and browser they are using?
