How do I know from which e-mail account/address the request was submitted?

    We received a filled in Request, but unfortunately we don't know who submitted the request and thus cannot contact the requestor.

    Do you have a possibility to see from which e-mail account/address a request was submitted?

    Unfortunately, it seems like the information on those fields were not submitted. They were blank when the submit button was clicked.

    You can check your form's email history to see if the autoresponder email was sent successfully.

    May I know if the form was embedded on a website? I see that there are fields that were required but are blank in your submissions. There might be a code conflict between your website that may have caused this problem.
