    All of our web pages that have a "JotForm" form embedded in them, no longer show the form.  The space where the form should be is blank.  Our last posted payment was on 6/14/15, so it stopped working sometime after that.  I have attacked one of the web page URL's below.

    I am really hoping you can fix this from your end ... I am totally stumped.

    Could you please email or call me so together we can get this fixed as soon as possible?

    Thank you,

    Annie Lutz

    The reason why this happened Annie is because your account is over limit with 10 secure submissions - which is the limit for free plan.

    You are also embedding your forms with a wrong code.

    Please change the embed code URL of your forms from:

    <script src="https://secure.jotform.com/jsform/1360826176">

    to:

    <script src="https://secure.jotformpro.com/jsform/1360826176">

    Now it is empty because you are calling the js (javascript) version of your form and that expects text as a reply, while if you go to the form directly, you will see the error: https://secure.jotformpro.com/form/1360826176

    The only way to have them shown now would be to upgrade your account to some paid plan. I suggest Premium if you do not need a lot of submissions per month, but it is always best to check our pricing page to see what suites you best.
