I checked and test this on your form http://www.jotformpro.com/MountHawke/MHREG_2015 but I could not reproduced yet the mentioned issue on my browser.
Can you please retry again. Try clearing also first your browser's cache. Perhaps it is just a browser cache caused problem.
Please let us know if the issue persist.
