I created the form and mapped the fields and the form does not go to Salesforce. I get the email confirmations.
When I go to integrations it shows connected to SF.
I tried it in preview mode and loaded the code on the website. Both times I received the email notifications, but the lead did not populate to SF CRM.

I do apologize for the issue that you are experiencing with your Saleforce integration.
Please remove the current integration from your form, clear your forms cache and create the integration once again.
Also ensure to check the box to add existing contacts
