How to change Page Title of my form

    I see there is a heading on top of the printed form..

    I suspect I was able to enter a description of the form when I created a few weeks ago.

    I can't find where I edit this description anymore.

    Please advise.

    If I understood you correctly you want to change your page title. You can do that inside of your Form Builder. Select Preferences and under General Settings there is Page Title option where you can change your Page Title.
