Email Notification: name showing twice

I receive two name fields rather than an name and email field from the response

The email is typed in correctly BUT I just get a name

Any ideas?

You seem to have a typo in the body of your Email Notification, where you are populating it twice with the name, even where you would want to put the email.

You can change the line to the right of Your E-mail Address to this: {yourEmail16}

That field will be populated with the email from now on.
