The form is filled in correctly but a duplicate name is given nstead of email address
No message is sent either

When i was checked your form, i saw dublicated area. You can fix it:
1. Click Emails -> Notification 1
2. You'll see dublicate area:
3. You can set to "Your Email Adress" on form field area.
After all that, you'll receive e-mail inputs from your form.
