As I tried to submit your JotForm, I was able to submit it without any issue.
The provided error message shall appear when the submitted form with thank you page is accessed or refreshed. Please try submitting the JotForm using its URL http://form.jotformpro.com/form/51456470156960 and see if you are able to submit the form. Since you have multiple upload fields on your JotForm, please allow some time to allow the files to upload into the server.
You may also consider using Allow Multiple and Maximum number of files options in the upload field properties so that the upload status can be shown and restrict users to upload more number of files.
Also, please try clearing the browser cache by following the steps provided in the guide below to see if that helps.

Same error repeatedly. As submitter, don't have access to Properties settings to allow multiple or set max number. Cleared cache each test in each different browser. Only browser that indicates upload percent is Chrome but timeout and error the same. Have chosen a single movie file each time and stayed within the size limitations (100MB). One test with MP4, another with MOV file.

I've tried submitting the form again and the submission went fine. Since it takes a while to upload the files, please try uploading a smaller video file (less than 5 MB) to see if you are able to submit the JotForm. Please do not refresh the window or try to resubmit the form until the thank you page appears.
