Create conditional fields

    I want to create a form where there is first a check box--do you have children. If the respondent answers yes, then a set of fields appears that ask for name, social security number, date of birth, etc. At the end of this group of fields there would be a button that asks something like "Add another child."  If they clicked that field then a duplicate set of empty "child" fields would appear.

     

    Can jotform do this?

    Thanks for contacting us.

     

    Yes, this is possible to do in JotForm. You will need to setup a Conditional Logic to show/hide field when a respondent selects Yes or No from your field. Then, you can use the Configurable List widget for the children details to have an option to add another children. I have created a sample demo for you. See screenshot below.
