Form link sends you back to the login page for jotforms.

When uploading, two of them work great, displaying the form.  However, the third one sends you back to the login page for jotforms...something my constituents will not have.   What Have I done wrong with this third form?

Contact US = good

Team registration = good

Team reports = the problem form

I should also say that it is almost right on the firefox browser but not on chrome or explorer.

I guess you are referring to the direct link of your form to load it on a browser: http://www.jotform.us/form/51446342602145

In order to get it, all you need to do is to login into your JotForm account, the click on MY FORMS, select your form, click on Edit Form, then on Embed Form:
