info not included in submission

    I created a photography contract for clients to electronically sign.  I have one that a client submitted but the text boxes that have the information they are agreeing to did not "print/register", it only shows the date and name.  I need to be able to print off all the information to have on file.  I need to prove what they agreed to.  Is there a way to trigger that info to show up?  What did I do wrong?  Here's a link to the form...

    At the moment the text in the HTML field is not displayed in the email by default,

    You will need to manually add the text in your email template in the compose section, preferably before the acceptance check-box.

    This way it will be a part of the email sent.
