An extra white space is showing up when I embed my form

Please see attached screen shot. Why is there an extra white piece?

Is your web page published? If so, can you provide the URL of your site so we can take a look at it?

If not, you may want to try to increase the width of your form to see if this helps.

Choose the "2015 FSA Enrollment Packet" .

The Spanish version looks the same way.

There are two forms in each packet that have the same issue.

I increased the form width from 700 to 800 with no change. Please let me know if I am doing something incorrectly.
