I did check your form "" and found that you have added two notification email alert and both of them are sending submission emails to email@example.com email address. Please check the screenshot below:
That is the reason why you are receiving two submission emails. I would suggest you to please go ahead and delete one of the email alert. That should solve your problem. The following guide should help you: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder
