Form appears blank after closing and re-opening

 não consigo editar um formulario depois que fecho o site quanto reabro o formulario ele aparece em branco, tb não consigo fazer upgread da minha co

I can not edit a form after closing the site as reopen the form it appears in white, tb can not upgread do with my co

I see that there are three JotForms in your account and the form titled "Formulário sem título" seems to be blank and doesn't have any fields. The other two forms titled "Escolha sua marmitex" and "Clone of Escolha sua marmitex" seems to be working fine.

If you are looking at the form "Formulário sem título", the form might not be saved successfully after adding the fields. As I check the form revision history, I'm not able find any edits on this JotForm. Please make sure that Auto-save option is enabled on your JotForm and also refer to the following guide that helps you in saving the JotForm.
