I would like to confirm this with you that the issue is happening on the following forms:

I ask since the names are similar to other forms, while no form matches the exact phrasing that you have used on the other thread.
Does the same issue happen on some other form as well?

Sorry I just realized these were not request for help in solving the but notifications that most of my issues were already solved, or showing me how to solve the problems on my end. So I apologize for the delay in clarifying your questions. The forms names are the ones shown above or St. Luke Spiritual Gifts and St. Luke Community of gifts. the trouble I am having is the formatting does not appear the same ad it used too (in 2 different color tones, rotating at every other questions) , but instead comes over like the attached. Jumbled and non aligned.Please let me know what can be done to make these look like the original form, when i need to turn them into a PDF. Thanks! Shelly

There was some updates made on the fillable PDF recently specially on the formatting, could be the reason why most of the issues are fixed.
About the other issues, jumbled, not aligned and colors, please try to inject this CSS codes to your form this might help (place it at the most bottom part). You have to create a new fillable PDF after that, and if you don't want to retain this style on your form, you can remove these code afterwards specially the part highlighted in yellow.

forms, and I don't ever seem to get the same result as the cloned forms you
did. (Which is what I am looking for) Can you please embed these codes into
both my St. Luke Community of Gifts form & the St. Luke Spiritual Gift Form
