The PDF seems to show same fields in different formats when exported to PDF

under the question # 80 (last Questions) the old form appears to show through (it shows #76-80) in the old format (which is how I would prefer the rest of the form to look if at all possible) I do need to be able to turn this into a printable PDF for some of our older members who do not have the internet.

This one is a bit confusing to me. If I understand this correctly, the issue is only shown on the PDF version of this form: http://www.jotform.co/form/51605655944865

When I checked the direct online version of your form (link above) I did not notice the same issue and I like to confirm if I have understood everything correctly just so there is no extra confusion.

This question address the same issue of non alignment in questions and where people can fill in thier choices. On the PDF the form does not align up as it should, I think I addressed this in the last message to you. Please let me know if that helps explain things or the issue I am currently having.
