The email address used for submission notifications is not in our bounce list. Have you checked your spam folder? The email notification is not properly setup: https://www.jotform.com/help/25-Setting-Up-Email-Notifications
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the "Name" field -or similar)
In addition to this, you may white-list our domain names, if the issue continues, as we use Amazon SES method for "email@example.com" sender.
