How to create a form that allows our patients to input a data?

 We are trying to use your forms on our website to make it easier for or patients to fill out and submit back to us. Unfortunately, I am unable to get the plugging that allows our patients to even type in any information. Is there any way you can tell me what I am doing or how to do that?

Instead of the text field, you should use the input fields in the form builder so that someone can fill your form. You can find these fields in the Form Tools and Quick Tools section:
