Estou com problemas / My form is disables

estou com problema e esta dando erro toda hora e é meu primeiro formulario

The reason why this has happened is because your account got autosuspended by our anti-phishing system.

Please note that form "Cadastro M2Solares !" is asking for username and password that is against our terms of use.

As soon as you either:

1. modify the form not to ask for the same or
2. delete your jotform (from trash folder as well)

that your account will immediately be un suspended and that your form(s) will again be active.

If you apply the change, but your account is still not enabled, do let us know and we would be happy to check it out for you.
