How to change the title that appears on the window tab?

Hi,

When clients click on the link for payment, the tab is called World Traveler  Vacations  and Cruises instead of my company's name. I think this is because this is the template Im using. Please assist. 

In order to change the page title, follow this steps shown in the animated image:

1) Go to MY FORMS, select your form and click on Edit form

2) Click on Preferences, and change the title:
