Form designer is not working?

The form designer feature keeps coming up stripped of styles and functionality. How can I get it to work as intended? I have tried it on multiple browsers and the same thing is happening in those browsers as well. 

I have checked in my end,  and the Form Designer is working fine on Firefox, Chrome and even IE - 11.
