If you wish hide or show fields depending on a user input, you can do that by using conditional logic. Here's a guide on that: http://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic.

Here's a quick guide that you can follow.

    1. Click "Conditions". Choose "Show/Hide a form field".

    2. Here you can set the rules and the actions. In this case, the rule is if the field regarding a Cosigner is equal to "Yes", then Show multiple fields at once. You can select multiple fields by choosing "Show Multiple" then press CTRL and click the fields to show, here's a guide on that.
